Paducah Symphony Orchestra • Benefit Truck Raffle • Complete Rules.  The purchase of a raffle ticket is $25.00.  Payments are non tax-deductible contributions.  Payments may be made by cash (cash sales MUST be in person only) check, money order or by Visa, MasterCard, American Express or Discover Cards.  Check or money orders should be made payable to Paducah Symphony Orchestra, attached to a completed registration form and mailed or delivered to Paducah Symphony Orchestra, 2101 Broadway, Paducah, KY 42001.  Phone: 270.444.0065.  Raffle ticket(s) will be mailed to the entrant within 10 working days after receipt of completed registration form and payment.  If an entrant’s bank does not honor the check or credit charge, the PSO will use reasonable effort to notify the entrant by mail or phone.  The ticket and entry will be declared invalid unless full payment is made prior to the drawing.  By submitting a completed registration form, the participants are accepting these rules for the PSO Benefit Truck Raffle as stated.  Completed registration forms must be in possession of the Paducah Symphony Orchestra, 2101 Broadway, Paducah KY 42001 on June 6, 2008 or before 5,000 registration forms are received, whichever occurs first, to be eligible for the June 7, 2008 Benefit Truck Raffle grand prize drawing.  The 2008 Benefit Truck Raffle will run from March 1, 2008 to June 6, 2008.  Purchasers of Raffle tickets must be 18 years of age or older.  Grand prize shall be a 2008 Ford F150 Truck or $20,000 cash, subject to these rules.  Names of the winners will be available to the media, all raffle participants and displayed on the PSO website (www.paducahsymphony.org) for 60 days after June 9, 2008.  An entrant’s chances of winning will be determined by the number of tickets sold and distributed.  No more than 5,000 tickets will be sold.  The Raffle will be closed at 5 p.m. CDT on June 6, 2008 or when 5,000 registration forms are in the possession of the PSO, or its representatives, whichever occurs first.  Any registration forms and money turned in after the closing of the Raffle will be returned to the buyer.  All registration forms will be recorded on a first come, first served basis.  The grand prize winner will be determined by random drawing on June 7, 2008 at approximately  8:00 p.m. CDT at the Floodwall Stage, 100 North River Front, downtown Paducah. The winning ticket holder(s) need not be present to win.  Winners will assume and be responsible for all licensing, registration, transportation and all other fees, and local, state and federal taxes, including without limitation, all applicable sales, use, luxury and income and special taxes.  PSO SPECIFICALLY DISCLAIMS ALL EXPRESS OR IMPLIED WARRANTIES, INCLUDING WARRANTIES OF MERCHANTABILITY AND FITNESS FOR A PARTICULAR PURPOSE REGARDING THE TRUCK.  If 2,000 tickets are not sold by June 6, 2008, PSO reserves the right to modify the Raffle and in lieu of awarding the 2008 Ford F150 Truck or $20,000 cash, will instead split the remaining net proceeds (income less expenses) equally between PSO and the Grand Prize winner.  All proceeds, less prizes and expenses, will benefit PSO, Inc.  Only one name may be stated on the registration form and the ticket, and payment or delivery of any prize shall be made solely to the named entrant.  The decision of PSO shall be final on all matters concerning the Raffle.  This raffle is void where prohibited by law.  PSO officers and administrative employees and residents of their households are not eligible to receive prizes.  The Kentucky Revenue Cabinet requires the PSO to obtain and provide the name, address and social security number of the winner before awarding a prize, and winner understands and agrees that receipt of a prize is conditioned on providing this information.  Social security number will be disclosed only as essential for compliance to PSO office personnel and to the Cabinet.  These rules are intended to incorporate and comply in every respect with applicable law, and shall be interpreted, and any inconsistencies shall be deemed amended to comply with applicable law.  KY Gaming License #1843
